Effectively examining human rights violations depends on collecting accurate, reliable and systematic information.

The Reporting & Documentation Center, developed by IHRO, is designed so that victims, family members, lawyers, health workers, human rights defenders and witnesses can report cases safely. Applications are recorded and undergo preliminary assessment in line with international human rights standards and scientific documentation principles. Each application may be assessed independently by medical, forensic-medical, psychological, legal and human-rights experts, according to the nature of the information and documents provided.

The application process
  1. 1Choose the appropriate form. Select and complete the form that matches the nature of the violation.
  2. 2Enter the information. Fill in what you know about the incident as accurately and completely as possible.
  3. 3Attach supporting documents. Optionally attach any documents you have. Submitting documents is not mandatory.
  4. 4Preliminary assessment. Your application is recorded. After an initial review it may lead to a request for more information, referral to expert assessment, archiving, or entry into the reporting process.

Torture & Ill-Treatment Incident Report

For documenting allegations of torture, ill-treatment, inhuman or degrading treatment, violence in custody, ill-treatment in prison, and excessive use of force by law enforcement.

Suspicious Death Incident Report

For documenting deaths in custody, prison deaths, unexplained or sudden deaths, deaths resulting from medical neglect, and deaths where a human rights violation is suspected.

For a preliminary assessment of people considered, on health grounds, unfit to remain in a penal institution — weighing existing illnesses, functional capacity, access to treatment, prison conditions and access to health services together.

Enforced Disappearance / Abduction Report

For systematically documenting enforced disappearance, abduction, secret detention and persons whose fate is unknown — weighing last known location, last contact, witnesses, digital evidence, official applications and CCTV footage.
